What Goes Into a Medical Chronology

A medical chronology converts a stack of records into a timeline a lawyer can argue from. Done properly it replaces the record for most purposes; done poorly it is a second stack of paper.

The core timeline

One row per encounter, in date order: date, provider, facility, presenting complaint, findings, diagnosis, treatment or medication, and the page reference. The page reference is not optional — it is what allows anyone to verify an entry in seconds.

The provider index

A separate list of every provider and facility in the record, with the date range each one treated and the volume of records attributable to them. It tells you instantly whose deposition matters and whose records are missing.

Pre-existing conditions

Entries that predate the incident, isolated and flagged rather than buried in the main timeline. Defence counsel will find them; you should find them first.

Treatment gaps

Periods where treatment stops and restarts, with the length of each gap. Gaps drive causation and mitigation arguments on both sides, so they belong on the face of the document.

Billing, where records support it

A parallel schedule of charges by provider, tied to the same page references, so the damages picture and the treatment picture cannot drift apart.

What a chronology should not do

It should not characterise, argue or diagnose. It should not fill silence in the record with inference. Where the record contradicts itself, both entries appear with their citations and the contradiction is noted — the conclusion is counsel's to draw.
